This informs me to prepare any modifications that may be required for your practice.
I also encourage you to keep me up to date with any changes to your health or personal situation that may affect your practice, on an ongoing basis.
All information provided will be treated in the strictest confidence and stored in accordance with Data Protection / GDPR legislation.
Data is held in Google Forms GSuite where it is stored securely
